Office
Location
Our office is located in T tower (Creekside Campus) at Providence
Alaska Medical Center in Anchorage. Patients are seen
in the office by referral only. Office hours are 9-5 Monday
through Friday. We are closed daily from noon to 1:30 PM.
Although our office is located on the Providence campus, Dr.
List practices at both Providence
and Alaska
Regional Hospital.
Office Philosophy
You
will be encouraged to be an active participant in your care,
or the care of your child. You will be provided with information
that will enable you to make informed decisions, and help
you follow our recommendations.
Telephone
Calls
Your
telephone call will be answered by an automated answering
service. There are only four prompts to chose from, so please
listen carefully for the appropriate choice. We have chosen
this system as it enables us to provide all of our patients
with shorter "hold" time and more efficient response
to your needs.
Prescriptions
Prescription
refills are to be requested Monday through Thursday between
9:AM and 4:00 PM. Allow 24 hours for your request to be filled.
When you leave a message on the request line, be sure to include
the name and location of your pharmacy.
Financial
Policy
Charges
for your care, or care of you child are your responsibility.We
do not bill insurance for office visits. Payment may be made
with cash, check or credit card. When
you leave the office, you will be given a receipt that will
be acceptable to your insurance company, to submit for your
reimbursement.
Insurance
will be billed for most procedures performed in the office
or hospital. Surgeries not covered by insurance require payment
in advance. Contact our office manager should you need to
make payment arrangements.
Requests for Medical Records
Please allow at least two business days for your request to be processed. Your records will be provided to you in an electronic format on a CD disk. Medical records have not been archived for patients who have not been seen since 2003.

Health Record system.
